258 Fingerprint Card (United States – FBI)

FD-1164 Fingerprint Form (United States – FBI)
The FD-1164 fingerprint form is an alternative FBI-approved fingerprint form used for U.S. police certificate requests.
Some U.S. agencies or organizations specifically request this form instead of FD-258, even though both serve the same purpose.
Applicants should always follow the instructions provided by the requesting organization to confirm which FBI form is required.
F-216C
The C-216C fingerprint card is an RCMP-certified fingerprint form used to obtain police clearance certificates for countries outside the United States.
This form is processed through the Royal Canadian Mounted Police (RCMP).
Some countries require fingerprints to be submitted on the C-216C card, while others may only accept their own country-specific fingerprint form. In these cases, ink-and-roll fingerprinting may be required
